Clinical Director Appraisal Form

All doctors must complete an annual appraisal form in order to fulfil the requirements for revalidation. The appraisal process will differ depending upon whether the NCCG is based predominantly in Primary or Secondary care and NCCG should establish how the national guidance for appraisal is being implemented locally.

It is recommended that, as part of the appraisal process, all Non Consultant Career Grade doctors meet with the Dermatology Clinical Lead /Clinical Director to discuss how their role and professional development fits with departmental developments and priorities.

The form entitled Clinical Director Appraisal Form (click here to view) is included to provide a simple framework for this discussion. This form and its recommendations should feed into the clinician’s appraisal, objectives and personal development plan.

Although Hospital Practitioner and GP Clinical assistants will usually have their appraisal undertaken by a Primary Care colleague, it is very important that their personal development requirements with regard to their dermatology experience are not ignored.

The aim of the meeting with the dermatology clinical lead and completion of the documentation is to facilitate this. We hope you will find it helpful.
